The picture you posted looks a little blurry, but I am certainly led to believe that it is Creeping Jenny. A few years ago I noticed it in my yard and didn't think anything of it since it looks so pretty. Little did I know it would soon cover nearly my entire yard and cover my pool supplies . I yanked it all out and it didn't return. I've heard from other people in my area that they had luck with doing the same thing. If you're afraid to use weed killer like I am, consider doing that instead of spraying your yard with chemicals.

This does look like Creeping Jenny to me too, it would have helped more if you could take a good picture of the patch and post it here, because everyone will come-up with what he/she knows on this picture and you will not get a direct response on it. what I will recommend in cases like these (where cause is not certain) is that you remove the grass from the patch with roots and over-seed and aerate the patch again and hope for the best, if you do not own an aerator and you lawn size is pretty small buy one of those manual aerators else you can rent one form the home depot tool rental.
